How to apply for NDIS Funding?

Application Process
The first step is to complete the Client Access Form, you can find it here. You will need to print it and complete Section 1 (pages 2 -12). Alternatively, you can collect a printed copy from our office and seek assistance in completing this from a staff member. Our location can be found here.
You will then need to visit your Doctor and ask them to complete section 2, which is the functional assessment and their diagnosis.
Your Doctor may request you visit a Clinical Psychologist or Psychiatrist to confirm the diagnosis, depending on the level of NDIS assistance required. They will provide you with a referral and the details of where you will need to attend this appointment. The Clinical Psychologist / Psychiatrist may need to fill in some of the paperwork that the Doctor is unable to.
Following this you will need to read Attachment A on page 26 to make sure you have completed and included all of the information required on the checklist. All of the details for proof of identity is included in Attachment B on page 27 and 28.
Once you have completed all of the required sections of the form as the "applicant" and included all of the supporting documentation including proof of identity and supporting documents from your treating Doctor, Psychologist or Psychiatrist, you are ready to submit your application.
Your can either send all of the documentation in the mail to the NDIS. Or the staff at KoaHanii will copy this and email it directly to NDIS which will see your claim lodged a lot faster. They will also provide you the originals back so you have a copy of all of the documentation.
